Premiere Patient ProgramIn an effort to help provide financial assistance to our patients, Dr. Gerilyn Alfe and her Chicago Smile Spa team are proud to offer our own version of a rewards program. This is also a fantastic option for patients who have no dental insurance, are self-insured, or have minimal dental benefit coverage. For a reasonable annual fee, all of these benefits are available for one calendar year.

• 2 Hygiene visits, which include 2 exams, one VELscope® exam, 4 bitewing radiographs & either 2 prophy or 2 periodontal maintenance visits, are covered
• 12-month duration
• Fee reduction of 10% off all treatment completed within a 12-month period
• Unlike insurance, we place no limitations on what treatment you choose- you can use it for fillings, whitening, veneers, mouthguards, Botox®, Invisalign®, etc.
• Fees are locked in for that 12-month period, regardless of any fee changes that may occur
• Earn one point for every dollar spent annually. Special rewards for 5,000 annual points and above
